USI provides this notice to our customers, website visitors, vendors, employees, contractors, and consultants so that you will know what we will do with the personal information, (including but not limited to Social Security numbers, personal financial and health information) that we may receive from you directly or receive from your health care provider or receive from another source that you have authorized to send us your personal information. We at USI are concerned about your privacy and assure you that we will do what is required of us to safeguard your personal information.

In addition to the disclosures in this privacy notice, if you are a resident of California then the additional provisions of Appendix 1 (the “Privacy Notice for California Residents”), or Appendix 2 (the “Privacy Notice for Job Applicants and Employees, and for their Emergency Contacts, who are California Residents”), may also apply.

This Privacy Notice is provided in accordance with, and is subject to, U.S. law. Personal Information collected from individuals located outside of the United States is subject to the provisions of Appendix 3 (the “International Privacy Notice”)

“Personal information” means any data that relates to an identified or identifiable natural person, including information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular consumer or household. Personal information includes, but is not limited to, Social Security numbers, personal financial information, and health information.

What Types of Information Will We Be Collecting?

USI collects information from you required both for our business and pursuant to regulatory requirements. Without it, we cannot provide our products and services for you. We will be collecting personal information about you from:

  1. Applications or other forms, such as name, address, Social Security number, assets and income, employment status and dependent information.
  2. Your transactions with us or your transactions with others, such as account activity, payment history, and products and services purchased.
  3. Consumer reporting agencies, such as credit relationships and credit history. These agencies may retain their reports and share them with others who use their services.
  4. Other individuals, businesses and agencies, such as medical and demographic information.
  5. Visitors to our websites, such as information from on-line forms, site visitorship data and on-line information collection devices, commonly called "cookies".
  6. Tracking technologies as well as third-party cookies and non-cookie technologies, such as IP addresses, cookie identifiers, hashed email, and website activity.

What Will We Do With Your Personal Information?

The information USI gathers is shared within our company to help us maximize the services we can provide to our customers. We will only disclose your personal information as is necessary for us to provide the insurance products and services you expect from us. USI does not sell your personal information to third parties, nor does it sell or share customer lists.

We may also disclose all of the information described above to third parties with which we contract for services. In addition, we may disclose your personal information to medical care institutions or medical professionals, insurance regulatory authorities, law enforcement or other government authorities, or to affiliated or nonaffiliated third parties as is reasonably necessary to conduct our business or as otherwise permitted by law.

This includes "NextRoll" and “Google Analytics” for the purposes of targeted advertising, cross-device tracking, as well as measurement and analytic purposes. You can decline to have personal data collected via third party tracking technologies by navigating to the settings feature in your browser and declining all third-party cookies or declining third party cookies from specific sites. For mobile, you may limit ad tracking or reset the advertiser identifier via the privacy settings on your mobile device. You can also opt out of seeing personalized ads from RollWorks (and other companies) using the NAI opt out tool here or opt out of Google Analytics using the opt out link here.

USI will keep your personal information for as long as it may need it to provide you with insurance services and/or to comply with its contractual and/or legal obligations.

Our Security Procedures

At USI, we have put in place reasonable measures consistent with industry and legal standards to ensure the security and confidentiality of customer information. We will handle the personal information we receive by restricting access to the personal information about you to those employees and agents of ours who need to know that information to provide you with our products or services or to otherwise conduct our business, including actuarial or research studies. Our computer database has multiple levels of security to protect against threats or hazards to the integrity of customer records, and to protect against unauthorized access to records that may harm or inconvenience our customers. We maintain physical, electronic, and procedural safeguards that comply with federal and state regulations to safeguard all of your personal information, especially Social Security numbers. Specifically, we protect the confidentiality of Social Security numbers, prohibit unlawful disclosure of Social Security numbers and limit access to Social Security numbers.

Privacy Rights in Certain Jurisdictions

Certain jurisdictions extend enhanced personal information rights to residents of or persons located in the jurisdiction. You may have some or all of the following rights in relation to the personal information we collect about or from you, depending on the jurisdiction and our reason for processing your information:

  • Right of access: You may ask us to confirm whether we are processing your personal information and the specific pieces of personal information we have collected and, if necessary, provide you with a copy of that personal information (along with certain other details).

International Privacy Notice

If you are in the European Union and the EU General Data Protection Regulation (GDPR) applies to our processing of your personal information, the following conditions apply to you:

Categories, Purposes, and Legal Basis for Collecting Your Personal Information

The categories of personal information collected by USI, and the purposes for which each category is processed, are specified in Appendix 1 and Appendix 2.

USI collects your personal information so that it can provide you with insurance services. Accordingly, USI’s legal basis for the processing of your personal information is that it is necessary for the performance of a contract to which you are a party or in order to take steps at your request prior to entering into a contract.

If you refuse to provide USI with your personal information, you may be unable to obtain insurance services from USI.

USI will not use any automated decision-making (such as profiling) with respect to your personal information.

Transfers of Data

USI is located in the United States, and all personal information we collect is processed by us in the United States. United States laws regarding processing of personal information may be less stringent than the laws in your country, but USI will hold and transmit your personal information in a safe, confidential and secure environment. USI will also disclose your personal information to third-party insurance carriers in the United States as necessary to provide you with insurance services. USI has entered into Standard Contractual Clauses with such recipients. USI will not disclose your personal data to any other third parties.

Your Rights with Respect to USI’s Collection and Use of Your Personal Information

You have the right to request from USI access to your personal information, and the rectification of inaccurate personal information concerning you. You have the right to obtain from USI the erasure or the restriction of processing of your personal information in certain circumstances, including when the data are no longer necessary in relation to the purposes for which they were collected or otherwise processed, except when USI is required by law to maintain or otherwise process your personal information, for the establishment, exercise, or defense of legal claims, or for the protection of the rights or another person.
